this is a declaration of function with argument int* that returns pointer to an array of int*:

int (*ptr(int *b))[5];
int i;
int (*pi)[5] = ptr(&i);
i = *pi[0];

The atomic_dec family of functions decrement (by one) the variable referenced by ptr in an atomic fashion. The *_nv() variants of these functions return the new value.
