I don't know if it will works but if you can access single user mode, try
- eeprom auto-boot=false
- eeprom nvramrc=true

Then try shutdown -i0 -g0 -y

1. enter root password
2. fsck /dev/rdsk/c0t0d0s5
3. reboot

Are you by any chance using a USB keyboard?

enter maintenance mode with root passwd and then at # prompt give init 0 that should take you to OK prompt.

USB keyboards can be problematic in this situation. Replace it with a regular keyboard if at all possible.

Hammer & Screwdriver

Login in maintenance mode and type init 0, now you will come down to OK prompt and you can boot from cdrom.

But I think no need to boot from CD and run fsck, you do the fsck from the maintance mode with command

fsck -Y

and reboot.
