Following may help you also.

awk -F, '{gsub(/.*\=/,X,$2);A[$1]=A[$1]?A[$1] OFS $2:$2} END{for(u in A){print u OFS A[u]}}' OFS=, Input_file

Output will be as follows.
Code:
1,1000,CedarparkHospital,john,doe,User001
2,User002,2000,steve,jobs,Providence
