What breaks inittab

Today there was a situation where processes running from inittab was broken. Can someone help me understand, how to find out, who might have stopped those processes? Or how does it get broken?

We may not be able to solve this, but please give lots more information.
What Operating System and version you you have?
Please post the contents of your /etc/inittab highlighting those processes which mysteriously failed.
Quite honestly about the only thing you could do with serious consequences is to kill process ID number one (init). This is all too frequent.

No offense meant, but this looks to me like an exam or lab question by someone who doesn't really understand inittab. As methyl said, more info would be helpful.

One bit of wild guessing: you can screw things up by removing or changing perms on your getty program.. I've done that to my students before.
