still getting the same error and more such as below :

Can you give us more information about the hardware configuration please.

There are two disk controllers (c2 and c3) and everything on C2 is working. A drive on c3 which appears to be a fibre channel drive is "unavailable".

{@everybody......just thinking out loud.....
Could this fibre channel drive on c3 be dual-tailed on c2 with c2 being the primary route and the FC driver seeing the same WWN on two paths has quite rightly locked out the secondary path (otherwise corruption would occur). What do all the experts on this forum think about that theory??}

This may not be an error at all???!!!???

Looks like FC disk to me, for which multipathing was never active (probably due not being supported or configured properly in scsi_vhci.conf)

Anyway, in some period of time someone removed the FC disk or FC zone, so that disk is no longer available to the operating system.

Reboot of host should remove that device, and it should not be there in format.
If you cannot reboot, you can just ignore it.

I would also like to point out that you should activate multipath only for devices that actually have it (ISCSI or FC disks), not internal disks.
It works, but that just bad practice (in my opinion).

I have reboot the server and the error is still there.

I have also try to plug out one of the fiber card which have no led light but it disconnect me from remote login. I have to restore the card and restart the system.

Also found a similar situation on unix.com/showthread.php?t=109823 this forum but with no solution.

I would like to see the evidence of the fc disk. Please do the following:

I suspect a problem with the fiber channel driver.
Google for Solaris fiber channel patch qlogic qlc driver
and follow the suggestions from Oracle.
