Help with Sed

I am trying to format the following text:
Code:
4873:Ken:Williams:Maple Dr.:(845)674-3849

to read like so
Code:
User ID: 4873
Last Name: Williams
First Name: Ken
Address: Maple Dr.
Phone #: (845)674-3849

Sort of this approach?

Code:
$ cat sample11.txt
4873:Ken:Williams:Maple Dr.:(845)674-3849

$ cat sample11.txt | awk -F":" '{print "User ID:\t"$1"\nLast Name:\t" $3"\nFirst Name:\t"$2}'
User ID: 4873
Last Name: Williams
First Name: Ken

Piping from grep command

The code looks like it would filter a text file with a single line of entry.
But what Im trying to do is extract one line of code via its User ID which is the first column of data and format it to print out on its own line.

database.txt

Code:
4539:Michael:Sven:Oak Ln.:3409395888
6529:Bill:Williams:Maple Dr.:8453458970

Wanted output

Code:
User ID: 6529
Last Name: Williams
etc....

Use grep and pipe the output to the awk statement mentioned by joeyg.

Code:
grep "^4539" database.txt | awk -F":" '{print "User ID:\t"$1"\nLast Name:\t" $3"\nFirst Name:\t"$2}'

Output:
Code:
User ID: 4539
Last Name: Sven
First Name: Michael

You can do it in awk itself!
Code:
awk -F":" '/^6529/{print "User ID:\t"$1"\nLast Name:\t" $3"\nFirst Name:\t"$2}' input_file

Thanks. I will try it on my VM.

I should have stated this before. I want the user to enter a User Id he/she wants to display and the program then uses that input to look for the pattern and extract it.

The code above works but is there a way to omit /^6529/ with the established $USER_ID?

Code:
awk -F":" '{if($1~USER_ID)print "User ID:\t"$1"\nLast Name:\t" $3"\nFirst Name:\t"$2}' USER_ID=$USER_ID input_file
