Try this...
Code:
awk '{
        /Port Index/ && port[++j]=$NF
        /Permanent Port Name/ && name[j]=$NF
        /NodeSymb/ && node[j]=$NF
}
END{
        for(i=1;i<=j;i++)
                printf("Port: %s Permanent Port Name: %s Server: %s\n",
                        port[i],name[i],substr(node[i],0,length(node[i])-1))
}' input_file

Code:
awk '{if ($0 ~ "Port Index:"){a=$3} else {a=""}} \
{if ($0 ~ "Permanent Port") {b=$0;}} \
{if( $0 ~ "NodeSymb:"){gsub(/"/,"",$7);c=$7}} \
{if (a != "")print "Port:", a, b, c}' /tmp/<your-filename>

Here is the output:

Code:
# awk '{if ($0 ~ "Port Index:"){a=$3} else {a=""}} \
> {if ($0 ~ "Permanent Port") {b=$0;}} \
> {if( $0 ~ "NodeSymb:"){gsub(/"/,"",$7);c=$7}} \
> {if (a != "")print "Port:", a, b, c}' /tmp/12

Code:
Port: 5 Permanent Port Name: 10:55:00:55:c9:70:2d:31 SERVER001
Port: 6 Permanent Port Name: 10:00:55:55:55:9c:55:c0 SERVER002
Port: 7 Permanent Port Name: 10:00:00:55:c9:4e:55:55 SERVER003
