if you have Python
Code:
#!/usr/bin/env python
for line in open("file"):
    line=line.strip().split("/")
    for item in line:
        print item.strip()

output
Code:
# ./test.py
mr.smith stvenson:
address #
tel no
personal data profile.
mrs.smith stevenson:
address #
tel no
occupation
personal data profile.

with awk
Code:
# awk -F"/" '{$1=$1}1' OFS="\n" file

If you have perl:

Code:
$
$ cat input.txt
mr.smith stvenson: /address #/tel no/ personal data profile.
mrs.smith stevenson: /address #/tel no/occupation/ personal data profile.
$
$
$ perl -ne '@x=split/[:\s]*\/\s*/;
> for($i=0;$i<$#x+1;$i++){print "$x[$i]",($i==0||$i==$#x)?"":":","\n"}' input.txt
mr.smith stvenson
address #:
tel no:
personal data profile.

mrs.smith stevenson
address #:
tel no:
occupation:
personal data profile.
