Thanks, I had just come back to post the resolution. You're right, it was a second entry in a file.

This entry was in /etc/pam.d/system-auth;

Code:

password    requisite     pam_pwquality.so try_first_pass local_users_only retry=3 authtok_type=

And this entry was in /etc/pam.d/passwd;

Code:

password   required     pam_pwquality.so retry=3

The second of the two lines is now commented out and all seems to be fine.
