C43 means "Diag boot cannot be accessed", you find a complete list of boot codes in your copy of the "Maintenance Handbook" or

IMHO you should not put the tape drive in your normal boot list. AIX maintains 3 different boot lists and "normal" means quite that - a normal boot. It is highly unlikely that you want to boot from tape under normal conditions. You can put several devices there because you might have a mirrored rootvg and you want to boot from either disk regardless of the other one being broken or not.
As i take it you have an old RS/6000 hardware box (otherwise 4.2.1 wouldn't boot at all) the following should do:
- put the key in "secure" position and power on.
- wait until the display shows "200"
- insert the tape with the mksysb
- turn the key into "Maintenance" position and press the reset button
The system should boot now from the tape into maintenance (single user, diag) mode. Follow the menus there to restore your system.
I found a
document describing the process, which seems to be accurate. To be honest i can't remember too well, as it is at least 10 years since i last booted a pre-Regatta system.
