To answer your second question as far as I recall the hard disk root filesystem is /dev/root (with one r) BUT if you are mounting the hard disk root filesystem having booted from removable media (DVD/CD or floppy) to a mount point (e.g. /mnt) then it is /dev/hd0root.

I am using Red Hat 2.6.23.1 with RAID 6 controller (card).
(When I use "cat /proc/partitions" I do not see the separate HDDs in the RAID, I see one drive for the RAID itself, as it is considered 1 large drive.)

All host controller errors are passed to the client drivers. Root errors are documented in hubd(7D).
In addition to being logged, the following messages may appear on the system console. All messages are formatted in the following manner:

No SOF interrupts have been received. This USB UHCI host controller is unusable.
The USB hardware is not generating Start Of Frame interrupts. Please reboot the system. If this problem persists, contact your system
vendor.
