I would add:-
- Possible router/firewall issues
- Excessive server load (either CPU meaning it never fully connected or too much network traffic leaving it unable to respond)
- DNS lookup failure, i.e. the DNS used by the server went off and the timeout was too long to allow the connection. The server will try to reverse lookup your connection for logging.
- Client side issue perhaps?
It's all a bit vague so I can only be vague in suggestions.

Am running the following commands on our server to generate a key for passwordless SSH, however we are getting 100% blank key files.
E.g. when looking id_dsa.pub or id_dsa they are 100% empty - the files get created, but contain no content and have a file size of 0b.

Recently I decided to intall second daemon of SSH for Winbind users.
I mean I have configuration AIX + Samba + AD and I can login to the server via SSH with AD accounts to 22 port without any problems.

I use the following command to clear the memory disk caches.

DESCRIPTION
ssh-keysign is used by ssh(1) to access the local host keys and generate the digital signature required during host-based authentication with
SSH protocol version 2.
ssh-keysign is disabled by default and can only be enabled in the global client configuration file /etc/ssh/ssh_config by setting
EnableSSHKeysign to ``yes''.
ssh-keysign is not intended to be invoked by the user, but from ssh(1). See ssh(1) and sshd(8) for more information about host-based authen-
tication.

/etc/ssh/ssh_config
Controls whether ssh-keysign is enabled.
/etc/ssh/ssh_host_dsa_key
/etc/ssh/ssh_host_ecdsa_key
/etc/ssh/ssh_host_ed25519_key

These files contain the private parts of the host keys used to generate the digital signature. They should be owned by root, read-
able only by root, and not accessible to others. Since they are readable only by root, ssh-keysign must be set-uid root if host-
