This looks more like a hardware error. But for sure you don't need the extension - install if you are booting from cdrom. That extension is only used, if you install from a jumpstart server to do an unattended installation.
What kind of hardware are you using?

Just got a solaris 8 blade 150 box with no users, only a root account. no one seems to know the password. I'd like to add one user. So I booted into single user mode via cdrom and added one. Can't seem to login using the new account, though. Here's what I'm using:

install-solaris invokes the Solaris Install program. Depending on graphical capability and available memory at the time of invocation,
install-solaris invokes either a text-based installer or a graphical installer.
The following minimum requirements for physical memory dictate which features are available during installation:
For SPARC machines:
128 MB
Minimum physical memory for all installation types
128 MB
Minimum physical memory required for windowing system
384 MB
Minimum physical memory required for graphical-based installation
For x86 machines:
256 MB
