I don't think 'raidctl' will work on a T4-1. Raidctl is for the older built-in LSI hardware raid controllers used on hardware like the T2000.
If it was a LSI like that, then as Bartus11 says, once a raid mirror is configured the mirror drive is 'hidden' from view.
This is a T4-1 which uses a different raid controller and different command set.......

I've already made the point that typical factory configurations have the first two slots mirrored when supplied to the customer and this appears to be what we've got here.
We've got to the point where several members are trying to help and speculating over the resolution of the problem but what we need is Kerrygold to post more output.
I agree with Bartus11 that we all wait for Kerrygold to get back online otherwise we are all just guessing.
My own experience with T4 series is pretty thin anyway.

I changed disk controller. Unplugged every disk, inserted another controller card, an IBM M1015. The disks are recognized in BIOS, and the controller card's BIOS shows up. But after that, I drop into GRUB and a prompt. That is all.

mfi -- LSI MegaRAID SAS driver
SYNOPSIS
To compile this driver into the kernel, place the following lines in your kernel configuration file:
device pci
device mfi
Alternatively, to load the driver as a module at boot time, place the following line in loader.conf(5):
mfi_load="YES"
DESCRIPTION
This driver is for LSI's next generation PCI Express SAS RAID controllers. Access to RAID arrays (logical disks) from this driver is pro-
vided via /dev/mfid? device nodes. A simple management interface is also provided on a per-controller basis via the /dev/mfi? device node.
The mfi name is derived from the phrase "MegaRAID Firmware Interface", which is substantially different than the old "MegaRAID" interface and
thus requires a new driver. Older SCSI and SATA MegaRAID cards are supported by amr(4) and will not work with this driver.
Two sysctls are provided to tune the mfi driver's behavior when a request is made to remove a mounted volume. By default the driver will
disallow any requests to remove a mounted volume. If the sysctl dev.mfi.%d.delete_busy_volumes is set to 1, then the driver will allow
mounted volumes to be removed.
