We tried to do the disk replacement today but with no success. We got stacked on the server not recognizing the new disk as CLAIMED. Below the procedure followed:

I was able last night to gather some more info about the OS and the condition of the disk. I think it needs to be replaced. As you will see from below is the alternate disk and not primary....
