note flag1 maybe some thing like "intel.*USB.*SATA" I would like to search contains flag1' section and get the id (it's a bug id)

WHAT? Some text to the spec would help us from guessing. Using wild assumptions, I came up with:

awk '$NF=="flag1" {split($1, T, ","); if (!X[T[1]]++) printf "%s%s", DL, T[1]; DL=","} END {printf "\n"}' RS= file
aaa,ddd,xxxx
