Sun Solaris 10 boot problem - ERROR: Last Trap: Fast Data Access MMU Miss

Hello,

We have a T5140 server with Solaris 10 and its suddenly throwing "segmentation core" when I login into the server and not showing any output for commands like df, mount etc. so I had to reboot the server to fix this issue. Please note that there's no boot disk mirroring.

But unfortunately the server would not come up (I think it loads kernel and after a while, throws the below error)

and comes back to OK prompt

I have gone through few threads that address this issue but none of them helped. I tried show-disks (no errors), probe-scsi-all with all OK except one error ERROR: /pci@400: Last Trap: Fast Data Access MMU Miss and I tried test-all which shows all OK and even tried reset-all but no luck.

Finally I tried boot -F failsafe which gives me a shell as below

Not sure how to proceed from here? should I try installboot or will it break further? Unfortunately, We have only hardware support (no OS support from Oracle). Not sure if it's a hardware issue at pci@400 controller/boot drive?

Can you boot from a CD or DVD? If so, you might have a corrupt boot disk.

If you can't boot a CD or DVD, you likely have bad RAM somewhere. If you have a memory expansion "mezzanine", that's the most likely problem. You can try booting without it: Remove a Memory Mezzanine Assembly - Sun SPARC Enterprise T5140 and T5240 Servers Topic Set

The server is at remote site so I will setup an install server on another machine and will try to boot this via net.

Meanwhile I ran some SP commands, please see below, It shows only about power supply but no other errors. Do they strike you anything else?
