Try
Code:
awk '
NF < 5          {T[3] = T[3] $1
                 T[4] = T[4] $2
                 next
                }
                {if (T[1]) print T[1], T[2], T[3], T[4], T[5]
                 split ($0, T)
                }
END             {print T[1], T[2], T[3], T[4], T[5]
                }
